New Patient Visit
Forms New patients will receive a packet of paperwork in the mail or you can complete your paperwork online once your appointment is scheduled. You must complete all paperwork and bring it with you to your appointment. You should also bring any films (including X-ray, MRI, CT, Bone Scan films/images) with you as well as your insurance card and photo ID. You should review your name, address and insurance information listed at the top of your encounter form for each visit to ensure accuracy. Please initial this information and notify us of any changes. Payment Patients are required to submit copay/co-insurance at the time of service. Co-pay is collected at check-in. Check-Out After your visit with Dr. Ratzman, you will be scheduled for your next visit if applicable and instructed on the next steps in your treatment. Any testing or further appointments will be scheduled at this time.
